Complete the admission process( Vital signs, Weight-MUST and BMI scoring, Body map - refer to TVN as needed, Waterlow Scoring, Dependency, PEEP, DNAR Form or any Advance Directives) Register/ Update GP Surgery and Pharmacy.
• Define priorities by planning and implementing individualized care plans. Review monthly or as needed.
• Reach out to patients , respecting their wishes towards their care. Establish a good trusting relationship with the patient and family members and involve them with the care management and treatment; 6 monthly care plan review.
• Maintain accurate electronic documentation in patients records. Inform patient and NOK regarding any referrals to other multi disciplinary team (e.g. SALT, PHYSIOTHERAPIST, OCCUPATIONAL THERAPIST and Clinics like OPMHS, AUDIOLOGY, DERMATOLOGY, EYE CLINIC ETC.)
• Assess patient conditions and liaise with GP. Administer medications orally, rectally, intramuscularly, subcutaneously, through PEG tube as prescribed; Obtain consent for COVERT medication administration;
• Maintain clear documentation in MARS with accurate running total of medications.
• Order and receive monthly medications; Update MARS Share information to patients and caregivers on diagnoses, chronic disease management, wound management and infections- MRSA, etc
• Provided necessary health education training for patients and staffs. Maintain effective communication - documentation, handover, meetings. • Address unit staff concerns, including staffing ratios, resource management, skill mix ,allocation of responsibilities and optimize workload management.
• Respond to life-threatening emergencies; completes incident reports and inform Deputy and Manager.
• Maintain confidentiality and GDPR.
• Complete mandatory training.
• Adhere to Company's Policies and NMC code of conduct
